Tire-Derived Product Survey

The California Integrated Waste Management Board would like to hear about your satisfaction level with the tire-derived product used in your grant project. Use the link below to access the online survey. If you are unable to access the online survey for any reason, you may download, print and mail in the survey. The information you can provide is very valuable to CIWMB. Thank you in advance for your participation.

Before taking the survey, there are a few points to keep in mind:

• Allow 10 minutes to take the survey.
• Do not close your Internet browser while taking the survey, as all information will be lost.
• Have your grant number on hand.
• If applicable, please gather any reports or statistical information/results regarding the tire-derived product to include in the section provided.
