Permit Applications

In addition to the application form, it is recommended that the applicant include the following information in a cover letter:

• Summary of project and/or proposed changes.
• List of attached and/or previously submitted documents with dates.
• Owner/operator certification.
• Copy to other agencies as required.
• Attached complete and signed application form.
• Send a copy to the local land use planning agency.

This review letter should contain the following information:

• Statement that application package was reviewed for completeness.
• Statement that application is complete and correct and accepted for filing or, if rejected, reasons for rejection with information on appeal of decision.
• Copy to the Board
• Don't forget to fill in the LEA sections of the application form.

Proposed Permits

Proposed Permit Submittal Cover Letter (LEA)

The cover letter for a proposed permit should contain the following information and attachments:

• Statement that this is a proposed permit (to differentiate from a draft permit).
• Summary of project and/or proposed changes.
• Completed and signed application form.
• List of attached and/or previously submitted documents with dates.
• Don't forget to fill in the LEA sections of the application form.
• LEA certification that LEA has reviewed application package and found it to be complete and correct and acceptable for filing.
• LEA CEQA finding
