Time Sensitive SWIS Inspection Form Update Information

This all LEA e-mail was sent on behalf of the Waste Compliance and Mitigation Program of the California Integrated Waste Management Board (CIWMB).

1. From this date forward please use New SWIS Data Entry Form (CIWMB Form #37) version 01/2008

This form (Adobe PDF, 70 KB) is used to obtain SWIS (Solid Waste Information System) numbers for new facilities and operations. The SWIS Data Entry Form, last updated in April 2005, has recently been revised. The new January 2008 SWIS Data Entry Form reflects the changes in business practices since April 2005 and allows for:

• Optional locator information such as two ways to provide a latitude and longitude, and assessors parcel numbers (APN), township and range information,
• E-mail addresses, and
• A place to indicate there are attached supplemental documents to support the SWIS number request. Examples of attachments may be required supporting documents and maps or site/operation information or operator/land owner(s) information and/or status of the site.

2. From this date forward please use Inspection Report Forms dated January 2007 only

Starting March 30, 2008 only inspection report forms that are dated January 2007 or newer will be accepted by the CIWMB. Any “older” date version inspection report forms submitted after March 30, 2008 will be returned to the LEA.

The new CIWMB Inspection Forms (dated 01/2007) have been available and in use since March 2007. If for some reason you have not converted over already, we suggest you stop using any older inspection report form versions immediately in order assure older forms are not sent to the office after March 30, 2008. We also encourage you to recycle all older versions of the hard copy forms and delete any old electronic versions of the forms you may have to reduce confusion.

Also, please make sure staff use the correct inspection report form type (Adobe PDF, 18 KB | MS Excel, 22 KB) for the activity type (facility or site or operation) being inspected.

Make sure that you have downloaded the free Adobe Reader 8.0 or newer version to be able to use, fill-out and save the new January 2007 fillable PDF inspection report forms.
