The scope of work for
the Sand City Dump Reconfiguration project is as follows (reference Granite
preliminary cost estimate and work scope at Attachment A and Alternative
"A" Construction Drawings at Attachment B):
- Prepare
and Submit a Supplemental Work plan (See Submittal Prior to Start Work Section)
- Review
and comply with all permit requirements (See Permit Compliance Section)
- Mobilize
equipment and personnel on-site
- Clear and
grub iceplant from cover of fill
- Construct
temporary site haul roads
- Excavate
waste from cell location and stockpile
- Provide
special measures for waste stockpiles as site conditions dictate (wind, rain, etc.)
- Excavate
100,000 CY cell and stockpile sand
- Coordinate
with Construction Manager on sampling
- Coordinate
with Construction Manager on HW response/disposal as necessary
- Excavate
100,000 CY waste from front of fill (150-178 feet) and place in cell
- Perform
Recycling and Reclamation as necessary
- Place
remaining waste over cell
- Grade
front of fill and backfill with stockpiled sand
- Cover
entire site with 2 feet of sand from stockpile
- Sculpture
Dunes into cover in accordance with Monterey Peninsula Regional Park District drawing (See Dune Restoration Drawings Attachment C)
- Perform
final clean-up and demobilization activities
- Perform
Invoicing for work performed
- Perform
project administration

Contractor
will submit a supplemental work plan in accordance with contract IWM-C3059, VII.
special provisions, section C. Compensable Services, item 6. Ensure
that the following items are included in the supplemental work plan:
a) Detailed
Construction Schedule
b) Detailed
Cost Estimate
c) Site
Specific Injury & Illness Prevention Plan (See Section J)
d) Site Access & Haul Routes (See Section D)
e) Site
Security Plan (See Section E)
- Submit
all payment and performance bonds, per paragraph 3-1.02, Section VI, General Conditions, of
the RFQ.

- Contractor
is responsible for all requirements contained in CIWMB Contract IWM-C3059.
- Ensure that a daily work log is
maintained in accordance with Contract IWM-C3059,
VII. Special Provisions, Section G. Daily Work Log.
- Conduct
daily/weekly site meetings to ensure work is progressing in accordance with schedule.
- Provide a
copy of the daily work log to the Construction Manager (The Environmental Engineering
Consultant will act as the Construction Manager, unless a CIWMB Engineer is designated
Construction Manager for a specified period).
- Prepare
monthly invoicing for payment in accordance with Contract IWMC- 3059, Exhibit C.

Comply with all permit conditions applicable to
project. Reference the following attached
permits:
a) Attachment D: Coastal Development Permit
b) Attachment E: Construction Grading Permit
c) Attachment F: NPDES Construction Activity Permit
To ensure that no traffic congestions or delays result
due to project activities ensure the
following items are considered in the Supplemental Work plan:
a) Establish haul routes for equipment
mobilizing, metal reclamation contractors, emergency
response vehicles, or other subcontractors, etc.
b) Establish
site entry control point(s) to maintain control of site access by visitors, subcontractors, etc.

Due to the location of the site and high-profile status
of the project in the community, security
will need to be addressed. Ensure that
the following items are considered in the
Supplemental Work Plan:
a) Erect temporary construction fencing
around perimeter of site or where necessary
to prevent public access during construction period.
b) Place signs at entry control point and
at every 100 feet of the perimeter that state
"No Entry, State of California Construction Project in Progress, Trespassers will be
prosecuted".
c) Ensure
that only established entry control points are used by visitors or subcontractors.
d) Provide
security during non-construction hours during critical points of construction, i.e. open cell
excavation, etc. (coordinate with Sand City Police).
e) Contractor
will take all precautions necessary to protect equipment and assets from vandalism and theft.

- Ensure stockpiles are accessible to
earthmoving equipment and do not block haul
routes.
- Ensure
that run-off from waste stockpiles is not conveyed into Monterey Bay. Mitigation
materials and measures will be available if such circumstances warrant
implementation, i.e. silt fences, absorbent, sand bags, etc. will be available.
- Comply
with all the conditions of approved Storm Water Pollution Prevention Plan (SWPPP) and Monitoring
Program required by the NDPDES Construction Activity
Permit. (See Attachment F).
- Ensure
that management of stockpiles is coordinated with the Construction Manager
and CIWMB Project Engineer for purposes of sampling and analysis.
- Perform
any measures necessary to prevent public access to exposed waste stockpiles.

- If hazardous waste containers or
substances are visually identified during construction
the following procedures will apply. (Reference: CIWMB Contract IWM-C3057, VII. Special
Provision, Section F. Refuse Removal and Disposal).
- If there
are any 55-gallon drum containers or other suspicious containers with or without labels or unfamiliar
odors emanating from containers or a site worker
develops any type of medical conditions (skin rash, nausea, etc.) after coming in contact with
materials, the operator or laborer will stop work and notify
the Site Superintendent.
- The Site Superintendent will notify the
Construction Manager, who will then initiate
notification procedures as necessary:
a) If
medical emergency use 911 system
b) Notify CIWMB project engineer
c) Hazardous waste (HW) subcontractor (sub) will be notified to mobilize and provide
Hazardous Categorization (HAZCAT) assessment
d) Provide
notifications to City, County Health, etc.
e) HW subcontractor will develop a work plan to deal with handling and disposal
of material
f) CIWMB
project engineer will approve work plan and authorize execution of work plan
- Construction superintendent will
coordinate with HW sub and provide any handling
of material necessary with proper direction from HW sub.
- Contractor will resume full
construction operations at the direction of the CIWMB project engineer. If possible, Contractor will resume any
other construction
activities at the site which do not impact hazardous waste operations in progress.

- Contractor
will assist, as necessary, the Construction Manager in obtaining any stockpile samples necessary as
outlined in the Site Sampling & Analysis Plan (Attachment G).
- Contractor
will coordinate with the Construction Manager on movement and handling of waste as it applies
to Sampling and Analysis.
- A map of all utilities for the site
will be obtained from public works and reviewed
for utilities effecting site work.
- Utility
Site Assessment (USA) will be contacted and advised of work to be performed. All USA requirements or City Construction
Permit requirements shall
be adhered to by the contractor.
- All
utilities (especially forced sewage main located adjacent to Highway 1) will be clearly marked.
- Contractor will brief all site
personnel on location of utilities and
strict avoidance
(especially heavy equipment).
- Access over utilities by heavy
equipment will be limited to specially prepared designated heavy equipment crossings, i.e.
"bridging" of crossings using steel
plate, etc.
- Excavators
and backhoes will not perform excavation within 25 feet of the surveyed and marked location
of any utilities including the forced sewage main.

Ensure that the following items are considered in the
Site Specific Injury & Illness Prevention
Plan (IIPP), which will be included in the Supplemental Work Plan (reference paragraph B, Section VII, Special
Provisions, of the RFQ):
a) The
contractor will prepare a site specific health and safety plan which will address site specific
hazards and measures to protect worker's health and safety per Title 8 requirements.
b) The
health and safety plan will, at minimum, address the following issues:
- Hazardous
material incident response procedures
- Heavy
Equipment Operations
- Open-hole
excavations/cave-ins
- Confined
space operations
- Site
trip and physical hazards
- Steep
slopes and loose soils
- IDLH
environments (low-oxygen environments)
- Personal
Protective Equipment required, e.g. at minimum, steel-toed shoes, safety
glasses, construction hard-had, Orange/reflective construction
vest, work gloves.

- Contractor
during course of excavating landfill will separate and remove the following items for
reclamation by a metal salvaging company (to be arranged for by the Contractor):
a) automobile
or vehicle carcasses
b) household
appliances including, refrigerators, washers, dryers, sinks, etc.
c) 55-gallon
drums or similar containers after HAZCAT is performed by HW Subcontractor or
as directed by CIWMB engineer.
d) furniture
including sofas, chairs, etc.
e) any
unidentifiable metal object larger than 1 cubic yard in volume.
- Metal
Salvaging subcontractor will remove all material from site and compensate contractor for
"as is" condition of the material. Contractor will not perform
any further preparation of material for reclamation.
