bind the company to the conditions and requirements in the agreement.
This form must be completed prior to fully executing the contract
documents.
Payee
Data Record, Std. 204, Revised August 2006 with updated
CIWMB address/fax number (Adobe PDF Fill-In, 185 KB)
Department of Finance form customized for CIWMB contractors and used by
CIWMB to prepare information returns (Form 1099). Contractors must
submit completed form to CIWMB in order to receive payment from the
State of California.

CIWMB contractors use this form to report products
purchased with CIWMB contract dollars that fit into one of the
